Your first visit to 1960 Physician Associates establishes a vital foundation for our relationship with you. During your first visit, we make sure to obtain important background information about your medical history and give you time to get to know your doctor.

When calling to set up an appointment for a new patient or newborn infant, please be prepared to provide us with the following information so that we may create the patient’s profile chart to set up an appointment. We will need the following information:
- Patients first, middle and last name.
- Date of birth (Patient)
- Home address (Where they currently reside)
- A local pharmacy to add on to patient’s profile if ever needing to send prescriptions.
- Valid phone number(s). (One you do not mind receiving notifications for appointments, concerns etc.)
- Valid emails (To receive referrals, paperwork, or to send over non emergent concerns for providers to respond to.)
- Insurance card – we request that you send a photo copy/screenshot of the front and back of the insurance card for so that it may be scanned into the patient’s chart for billing purposes.
- Information on the primary card holder of the insurance – name, date of birth and relation to the patient.
- Please note that if the insurance is for a newborn OR is an HMO plan that requires a primary care physician (PCP) to be designated, please call your insurance before your appointment to add your designate Doctor as your PCP. Then, contact our office and provide us a Reference number from that phone call (ask the representative of the insurance company for this).
